We offer extended weekday hours and Saturdays for Same Day sick appointments.
What insurance plans do you accept?
We accept all major insurances except for Medicaid. Please see our Insurance page for the individual companies we work with.
How do I obtain prior authorization for my medication?
If your medication request is turned down by your insurance company because it needs to be authorized, please call the office with the pharmacy name and we will process your authorization within 72 hours.
What forms of payment do you accept?
We accept checks, cash, and major credit cards.
What types of medical testing are performed in your offices?
At Forest Healthcare we perform a wide array of tests including blood work, bone density screenings, and EKGs.
Do any of your doctors see children?
Cindy Chang, MD is a Board-Certified Allergist and Immunologist, treating patients of all ages for asthma, environmental allergies, hives, food allergies, drug allergies, eczema, anaphylaxis, sinusitis, or immune deficiency. Our other physicians see and treat patients 18 years and older.
What is your protocol for referrals?
The referral line is answered 2 – 3 times a day and referrals are processed within 72 business hours.
The doctor will not renew my medication because I haven’t been seen within the last 3 months. Why is that?
Standard medication protocol dictates that patients who are on continuing medication(s) are to be monitored every three months unless you have made other arrangements with your physician. If your medication has been denied for a renewal because you haven’t been seen recently, we will renew your prescription for the short amount of time needed to allow you to see your physician.
If I see more than one Forest Healthcare physician, will other Forest Healthcare doctors have access to my medical records?
Yes, our computerized electronic health records system allows Forest Healthcare physicians to update your records instantaneously and share information.
Can I see more than one Forest Healthcare physician in the same day?
The only insurance company that will allow a patient to see more than one doctor in a practice in a day is Medicare. All other insurance carriers prohibit a patient from seeing multiple doctors in the same practice on the same day.
